Include your credit
card payment information (Visa,

Registrations received by fax are for
credit card payment only. To avoid
duplicate charges to your credit card, fax your registration form only once.

Payment
Registrations will not be processed
without payment. All registrations
must include payment by check or
credit card. All fees are in US dollars.
International registrants must remit
payment in US funds drawn on a US
bank.
Note: An additional $40 billing fee
will be applied to any registration that
requires processing of purchase orders
and/or invoices.
Confirmation
Registrants will be e-mailed
confirmation of registration within one
week of registration.
Cancellation
Notification of cancellation must be
submitted in writing. Cancellations
received by October 18 will be subject
to a $75 cancellation charge, $100
after October 18.
No refunds will be made for cancellations received after November 1st.
Substitutions are allowed at any time,
but requests must be made in writing
to ISSTD Headquarters.
Registrants
who fail to attend the ISSTD 25th
Anniversary Annual Conference without
prior notification of ISSTD will be
responsible for full payment. |

Important
ISSTD Membership Information
The conference registration fee does
not include ISSTD membership. We recommend that you join ISSTD, thereby qualifying for the discounted conference registration fee. Just click the “Join ISSTD” menu button above and become a member to qualify. To qualify for the discounted member
registration fee, your membership must be current.
ISSTD members who renew at the
conference for 2010 will receive
membership status at the conference.
